Liability Waiver By registering my child(ren) with the Lincoln Way Area Water Polo, I agree to participate (or allow my child(ren) and family members to participate) in the Lincoln Way Area Water Polo, and hereby release Lincoln Way Area Water Polo, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Lincoln Way Area Water Polo including travel to and from training sessions, water polo games or other scheduled team activities. I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participating in the Lincoln Way Area Water Polo program. Medical Release Waiver I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any supervisor, coach or other team administrator associated with the Lincoln Way Area Water Polo to seek and give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment. I hereby waive, release and forever discharge Lincoln Way Area Water Polo and associated supervisor, coach or other team administrator from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in Lincoln Way Area Water Polo activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child(ren) is (are) physically fit and capable of participation in all Water Polo Team activities. Centennial Pool Waiver WAIVER & RELEASE The Undersigned, his/her agents, attorneys, legal representatives, insurers, heirs and personal representatives (whether signing on behalf of himself/herself or on behalf of a minor for which he/she is legally responsible), hereby releases, waives, absolves, indemnifies, remises, and forever discharges The Village of Orland Park, its trustees, board members, officers, directors, employees and agents of and from any and all liability, claims, demands, liens, actions, causes of action, rights, rights of action, costs, expenses, duties, obligations, damages, remedies, responsibilities and liabilities, of whatsoever kind, nature and/or description, direct or indirect, at law or in equity, in contract, tort, or otherwise, which the Undersigned ever had, now has, or hereinafter can, shall, or may have or assert against The Village of Orland Park, its trustees, board members, officers, directors, employees and agents, for or by reason of any matter, cause or thing whatsoever (including without limitation injury, illness or death), arising out of or in any manner related to or connected with participation in any activities which are to take place upon Village Property, or other Village facility(ies), and/or use of any equipment located upon Village Property, whether or not such equipment is owned by the Village. The terms of this Waiver and Release are contractual and not a mere recital. The governmental authorities may have determined to permit athletic practices and competitions to resume, notwithstanding the continuing hazards posed by the COVID-19 virus. Neither Lincoln-Way Area Water Polo (LAW) nor its coaches, officers, board members, representatives, agents, administrators, referees, table workers or volunteers possess the medical expertise to determine whether it is safe to resume Water Polo Activities or to assess the degree of risk that you may be undertaking, should Athlete decide to do so. LAW urges you to consult publicly available information, such as that provided by the Centers for Disease Control, your personal physician and/or your local governmental agencies, so that you can make your own independent judgment as to the degree of risk that you will be undertaking should you decide to engage in water polo activities. Code of Conduct Form Lincoln-Way Area Water Polo (LAW) Player and Parent CODE OF CONDUCT As a member of LAW, each player will be expected to conduct himself/herself in accordance with the following provisions at practices, games, social activities and all other occasions where the member is representing the club: A. LAW members shall exhibit good sportsmanship at all times, in and out of the pool. B. LAW members shall follow the coaches, officials and other administrator’s rules and instructions at all times. C. LAW members shall behave in a responsible manner, including absolutely no self-phone use in the locker-room, not using or possessing alcohol, tobacco, illegal drugs, banned substances or fireworks. D. LAW members shall respect and take care of all property and facilities whether the property belongs to the member, a teammate, opponent or any other person. E. LAW members and their guests shall not use profane language or be disrespectful to any other person. F. LAW members shall not participate in horseplay, running on deck, or any other activity that would create a danger to themselves or others. G. LAW members and their guests shall respect the officials and their authority during all games. Failure to comply with this Code of Conduct will result in the following disciplinary actions: A. A disciplinary letter will be sent home which explains the misconduct. Prior to the member returning to practice and/or game, the disciplinary letter must be signed by the member’s parent/guardian and returned to the Coach. B. In the event of repetitive and/or excessive misconduct, the coaching staff and/or the Board reserve the right to decide upon and enforce an appropriate consequence for the member’s misconduct. C. If the Board believes that suspension or expulsion from the club is an appropriate consequence based on the member’s misconduct, the member’s parents/guardians, the member, and the coach must meet with the board to discuss the misconduct prior to the suspension or expulsion being enforced. Any expulsed player will not receive a refund. The Board’s decision is final. The LAW Board and the coaching staff endorse and support this Code of Conduct. The coaching staff is responsible for implementation of this Code of Conduct. Any questions concerning the policy or actions that result from this Code of Conduct shall be directed to the Board. We, the member of Lincoln-Way Area Water Polo (LAW) and the member’s parent/guardian, agree to abide by the LAW Code of Conduct as set forth above. We understand that if the member violates any provision of the Code of Conduct, the member will be subject to the disciplinary actions described above. Failure to sign this form will prohibit your player from participating in LAW. CONSENT TO PARTICIPATE - REFUND POLICY - PHOTOGRAPHY WAIVER I/We the parents or legal guardian of the player listed, do hereby give consent to his/her participation in all LAW activities during 2025. I understand that I or my minor child may be photographed or videotaped while participating in these programs. I give permission for photos and/or videotape of my minor child to be used to promote the “Lincoln-Way Area Water Polo” Club and such photos and videos will be the property of “Lincoln-Way Area Water Polo.” Processing Fee of $30 for cancellations by June 10, 2025 NO REFUNDS after June 15th, 2025.
